SLSQ U18 Communication & U18 Travel NON-Consent Forms

Surf Life Saving Queensland is committed to the safety of all members in our association. To ensure that Clubs and Members are adherent to the provisions of the SLSA Child Safe Policy, SLSQ has created forms for Clubs to implement to ensure that the provisions of the policy – specifically sections 4.6 (Use of electronic or online communications) and 4.13 (Transporting Children) are noted and managed. This form is for any members (parents or guardians) that DO NOT want any U18 Communications or U18 Travel with/of their child/ren that has not been authorised by them.
